fixed some styling and issue with default values

adjusted some of the survey styling, and made edit questions stronger by ensuring that the question form has been loaded before pushing all the values to the form. Made sure for a multiselect that default value showed correctly

30 day trial modifiations. Refactored Access helper and created a new License helper. Removed ansible/License module. Created a new license viewer that allows admin user to update the license key. Nag message at login now differentiates between admin and non-admin user.
